How to Get Your Connecticut Driving Record

To get your Connecticut driving record by mail, follow these steps:

  1. Complete this records request form
  2. Fill in section 1
  3. Complete the "requester" section
  4. Include a $20 check made out to "DMV"
  5. Include copies of 2 forms of identification (at least one should be a photo ID)
  6. Mail the form, check and identification to the following address
