How to Get Your Delaware Driving Record

To obtain your Delaware driving record by mail follow these steps:

  1. Complete your request form
  2. Have the request form notarized (be sure to sign the form in front of the Notary Public)
  3. Include $15 fee
  4. Include a self-addressed, stamped envelope
  5. Mail the form, fee and envelope to the following address
