You have three options for requesting your Massachusetts driving record: in person, by mail or by telephone.
In Person
Visit any full service Massachusetts Registry of Motor Vehicles location and pay the $15 fee by cash, check or money order.
By Mail
Send a written request that includes your name, date of birth, license number, Massachusetts address and the $15 fee, made payable to the Registry of Motor Vehicles, to:
