How to Get Your Montana Driving Record
You have three options for requesting your Montana driving record: in person, by mail or online.
In Person
Complete a driving records release form, have it notarized or bring a stat issued photo ID with you along with a $4 fee to the following address:

Complete a driving records release form and have it notarized or attach a copy of your driver's license, ID card or passport. Mail it along with the $4 fee and self-addressed stamped envelope to the following address:

Visit the Montana driving records service, select public user, follow the instructions to complete the request and pay the $6.50 request fee.
