You can obtain your Utah driving record by providing your complete name, date of birth, Social Security number and Utah driver license number to any Utah driver license office along with a $4.25 record fee.
If you are living out of state, simply complete the Utah MVR Request form, have your signature notarized and mail it along with your $4.25 fee to the following address:
